How tor refer your patient

There are four procedures that you must follow to refer a patient to a ChronicCure Therapy Clinic.

1. Evaluate the patients muscular pain as you would normally do.

2. Make sure the patient meets criteria and has no contraindications for Sonocur® Extracorporeal Shockwave Therapy Treatment.

3. Complete the "ChronicCure Extracorporeal Shockwave Therapy Treatment Request Form" for your patient to take to the ChronicCure Therapy Center.

4. Contact us to schedule an appointment at one of our ChronicCure Therapy Centres
